| A note from Lilly When it came to
that difficult time in our 43 years of married life together, to find a new
home for my spouse for assisted living, I found the most wonderful home,
Autumn Oaks in Slinger, Wisconsin.
He passed away one year later, but it truly was his home. I
actually became jealous of his receiving such loving care. Autumn Oaks
has a caring, patient love from the heart.
As for the staff, every time I would visit my spouse (3 to 4 time a
week) the care-givers greeted me with a cheery hello and a hug.
But Autumn Oaks offers much much more. It is a home that gives
their residents spiritual guidance, communion every Sunday, and services
every week for residents of all faiths.
The activities are a pleasure for the residents. Exercise
programs every morning, baking or crafts, painting in spring, trips via
handicap van or bus and lots of bingo. On weekends the residents can
chose from movies musicians coming in, or just having a quiet sleepy
day.
My spouse was at home there and all the residents and staff helped him
to die peacefully. |

Our aunt/sister Hilda Brodzeller has been at Autumn Oaks for almost two
years. She is ninety-three and in a wheelchair. The staff
here is very friendly and caring. We feel very comfortable that she is
well taken care of in a safe, homelike atmosphere. We would highly
recommend Autumn Oaks for the care of your elderly loved ones.

Dear Ann,
I am Richard Bielinski - the trustee for Joseph Bielinski, age 85.
We transferred Joe from Hartford Care Center. I was impressed by your
staff and services offered. My faith in your organization has exceeded my
goals for Joe. Loving care, freedom to think for themselves, the homelike
setting, the food all have helped Joe's attitude and general health. Keep
up the good work!
Your personal interest for the patients is outstanding

Dear Ann,
I could not be happier with the care Adeline received at Autumn Oaks.
The warm environment combined with friendly, caring staff makes your
facility a real home for the residents. I'm glad to recommend Autumn Oaks to
anyone who needs nursing care for a loved one.

Dear Ann,
I would like to thank you and your staff for making
my mother’s transition to assisted living a positive experience. After an
infection in her foot required surgery, it was apparent that she could no
longer live at home by herself.
The surgery and recovery time in the hospital was a
difficult time for her. Moving directly to assisted living could have added
to the difficulty, but you and your staff gave her the attention and care
she needed to overcome her problems. The daily bandage changing and loving
support you provided during the long healing process lifted her spirits and
provided the positive atmosphere she needed to get back on her feet.
I would also like to thank you for your continuing
attention to my mother’s diabetes needs. Her daily insulin and diet needs
require constant monitoring. Your work with her doctor has her blood sugar
under better control than it has been for quite a while.
My mother has adapted well to her new environment
and enjoys the daily activities now available to her as part of your
services. Thanks again to you and your staff for your outstanding work.
Sincerely,
Jim Mechenich |

Dear Autumn Oaks Family, To the Staff -- Ann, Al, Nancy, Denise,
Sandy, Chris B. and all the CNA's who showed such love for our mother, Ella,
-- thank you from the bottom of our hearts for the wonderful care she
received while living at your residence for more than three years,
Through all her ups and downs you were there for her. How she loved
the pets and children (especially the babies)! The opportunity to play
chimes was so stimulating for her! Her time spent on the deck was
always refreshing since she loved the outdoors! It was great that
Catholic services were available to her! God Bless you All! Hope
to see you soon!
To all the residents -- thank you for your concern about our mother,
Ella, whole she lived at Autumn Oaks with you. Having spent much time
with you I realize what great folks you are! I hope to come visit you
sometime again
Sincerely,
Grace Tennies and Family
