Autumn Oaks is a CBRF serving the needs of adults over 50 who have minimal to moderate physical or mental conditions that prevent them from caring for themselves. Autumn Oaks strives to create a sense of community with and for those who call it home. Combining supportive care with self-reliance, the Autumn Oaks concept enhances the concept for enjoying a vigorous life at any age. We offer the following advantages to our residents:
Our goal is to assist our residents in maintaining the maximum level of independence of which they are capable. We provide assistance with personal care, nutritional needs and socialization. Each resident is assessed by an RN to determine his or her needs and to adjust as those needs change. The Care Staff provides assistance in daily living in a family atmosphere. The activity coordinator and the RN provide specialized activities to stimulate mental capabilities and supervises Care Staff in assisting with physical activities. Activities also include homemaking activities, community outings, intergenerational activities and religious services. Residents are encouraged to participate in planning special events as well as meal planning. Meals are prepared by the Care Staff/Cook and are served restaurant style. Families are encouraged to be involved in all aspects of the residents life. Respite services are available to give at home caregivers a break. Respite stays can be from a few days to a month.
